The Family Therapy Center Appointment Request form collects your information solely for the purpose of providing access to our services in a convenient and timely manner. All information, except for your name and telephone number, is voluntary and users are not required to fill out any other portion of the form. We ask that you provide your name and telephone number so we may contact you about scheduling an appointment.

All information is confidential and available only to the staff of the Family Therapy Center. All clinical records are confidential are not shared with any other persons except in cases of child or dependent adult abuse/neglect, threats of harm to self and others, court orders, or with the client’s written permission.
